Nomination time for Order of Ontario
The Order of Ontario award was created in 1986 by the provincial government to recognize the highest level of individual excellence and achievement in any field. Since 1987, three hundred and seventy-three people have been honoured.
On March 31, the Honourable James K. Bartleman, Lieutenant Governor of Ontario awarded 27 people the Order of Ontario for 2003.
Perhaps you know of someone deserving of recognition? If so the provincial Ministry of Citizenship and Immigration is looking for new nominees for the 2004 Order of Ontario. The deadline for nominations is May 28.
Nominees must be residents or former long-term residents of Ontario. No elected federal, provincial or municipal representative may be appointed to The Order while in office. For example, former Ontario Premier Bob Rae was awarded this year, but is no longer an elected official.
No person shall be appointed to the Order posthumously unless their death occurs after their name has already been selected by the Advisory Council. The Order is not awarded for acts of bravery, and self-nominations will not be accepted.
All nominations are considered by an Advisory Council made up of the Chief Justice of Ontario, the Speaker of the Legislative Assembly of Ontario and the Secretary of the Cabinet and other distinguished members of the Order of Ontario. The Chief Justice serves as Chair of the Council. The Lieutenant Governor of Ontario is the Chancellor of The Order of Ontario, according to the government’s website.

Each nomination must include all of the following information:
1.) A completed nomination form.
2.) Brief summary (100 words maximum) indicating how long you have known the nominee and the reasons you believe this nominee is a worthy candidate.
3.) Detailed description of nominee and his/her achievements including background/history of the nominee.
4.) Written testimonials from at least two additional persons who can attest to the value and impact of the nominee’s achievements.
5.) Additional material including material in support of this nomination (i.e. other testimonial letters that add substance to the nominee’s achievement, publications, media stories, tributes etc). Only include additional materials that provide new information and insight into the nature of their accomplishments for review by the Advisory Council.
Please list all material enclosed to support this nomination. All material received must be in 8.5 x 11 format. All material is copied in black and white for the Advisory Council members; therefore do not send in multiple copies or bound versions. Do not send originals of important or official documents or photographs, as they will not be returned.
