Making the transition: from hospital to homecare

Hospital stays, no matter how short, likely leave most of us feeling more aware of the changes to our bodies brought on by aging and our physical limitations.

This can be even more pronounced for your elderly loved ones, even if they are seasoned veterans of hospital visits or they have a good positive attitude towards life in general. Personally, I think opening your eyes in the morning is the best possible start you can have to your day — and it sure beats the alternative!

However, chances are they may find it a bit overwhelming.

Your elderly loved ones have to deal with the implications of illness or recovery from injury, new medications and their side effects, rehab needs and more. They need to face the reality that things just aren’t the same. They also have to consider what to tell friends and families and deal with their reactions. For younger family members, this may be the first time realization creeps in that the end may be coming sooner than they thought.

Going home also means leaving behind the hospital services and supports that they have become accustomed to, and indeed depend on. This dependence may be temporary, but sometimes may be more permanent and an ongoing care assistance plan may be required.
